INSURANCE PROVIDER FOR:

I will provide an invoice as an out-of-network provider for you to submit to your insurance carrier. You may want to ask them the following questions:

• Does my insurance coverage provide for mental health benefits?
• How many sessions per calendar year does my plan cover?
• What is my deductible and has it been met yet?
• What would my co-pay be to the therapist?
• How much does my plan cover for an out-of-network provider?

REDUCED FEE: I have a few reduced fee slots available.

PAYMENT: Cash or check are accepted at the beginning of each session.

CANCELLATION POLICY: I require a 48 hour notice prior to your session if you must cancel a session.  You will be charged full fee for a missed appointment.
